This physician-directed, nurse-monitored program will coordinate care for patients who have had or are at risk for heart failure. Working one-on-one with patients, dedicated cardiac nurses will address a variety of issues including proactive screenings, dietary sodium restriction, exercise, symptom management and care plan navigation within the hospital system.
RWJ Hamilton Foundation Vice President Karla T. Stein says, "Our annual golf classic is a healthy, fun way to make a positive difference in our community. We are extremely appreciative of everyone who participated and all those who helped make the event such a success."
